Cốt truyện xoay quanh Dương Thái Vi, một nữ pháp y chuyên khám nghiệm tử thi tại thành Hòa Dương. Cô có hôn ước từ nhỏ với Phan Việt, người được mệnh danh là "thiên hạ đệ nhất mỹ nam". Biến cố xảy ra vào ngày đại hôn khi tiểu thư nhà giàu Thượng Quan Chỉ (người thầm yêu Phan Việt) cưỡng ép tráo đổi gương mặt với Dương Thái Vi. Tuy nhiên, ngay trong đêm đó, "Dương Thái Vi giả" (thực chất là Thượng Quan Chỉ) bị sát hại. Dương Thái Vi thật may mắn sống sót trong diện mạo của Thượng Quan Chỉ. Nghi ngờ Phan Việt là hung thủ đứng sau vụ án và nỗi oan của gia tộc mình năm xưa, cô giả làm Thượng Quan Chỉ để tiếp cận anh nhằm điều tra chân tướng.
